Twins, with identical names?

Any ideas why this unfortunate young man should have his death recorded twice? Neither entry is an insertion.

I would imagine some sort of attempt by the local reg office to either correct a mistake or submit a missed entry to the GRO which was then messed up at the GRO.

If you google his name there's photos of his gravestone, but I think it may be illegible!

Thank you, Merry! I may well have my own notes about the gravestone. He is no relation (well, unless 18th cousin 17 times removed) but I was curious about a gap of over 3 months in the recording.
