I have come across a marriage in 1964 which is a little strange

Philip A. B. SURNAME1

to

Freda K. SURNAME2

and

Kate W. SURNAME2

all under the same reference.

Who did he marry?
Would it be a name change?

Usually it just means the name the bride (in this case) signed differently to her declared name on the certificate, so both got indexed.

So she may have given her birth name but always been known as the other name and signed as such?

Possibly. I wonder if "Freda" was originally Winifred explaining why F changes to W?

I can see it now, Thanks Merry - it's just a first name inversion probably then.
